We take all patient feedback seriously, and you have already been directly contacted by one of our medical directors in more detail with regards to the concerns you have raised. We have carefully reviewed your concerns, the consultation notes, and the actions taken by the GP. Accuracy, detail and Integrity of Medical Records and Letters Having investigated in detail, we believe that your GP undertook a thorough assessment, which typically includes a detailed history of patient symptoms to facilitate the provision of a referral letter containing accurate and complete information of the clinical presentation and problem. Every medical consultation within a primary care setting will start with a history being taken from the patient about the presenting problem. This history, for example, will always need to include a timeline of the symptoms being experienced by the patient, because the timing of symptoms is required to form an accurate diagnosis and thereafter appropriate management plan. This history is then documented in the clinical notes and should be done so accurately. As a medical practice, we are bound by strict ethical and professional obligations to ensure that any documentation provided—including within referral letters—is accurate and aligns with clinical findings and history elicited during a consultation. Providing inaccurate or incomplete information may breach both ethical standards and legal obligations. GDPR We would like to reassure you that the GP acted entirely within the remit of GDPR and our privacy policy. To fulfil any referral request in general, it is necessary to collect and include relevant clinical details, for example, the duration and nature of symptoms. A referral letter that omits essential clinical information risks being incomplete, potentially leading to suboptimal and potentially unsafe care from the specialist. The collection of information required in taking a patient history was not excessive but rather a standard part of a comprehensive assessment. Referral letters which do not contain adequate information may be rejected, resulting in further delays to your care. Service Provided We believe that the GP conducted a detailed history and clinical assessment to provide the safest and most accurate care for you. We have also reviewed communication with the admin team as well as with the GP you consulted. The doctor communicated regarding her decision (as referenced in your feedback) in a timely fashion, and in a manner which we reviewed as appropriate and clear. We do truly regret that you feel dissatisfied with the service provided and we of course understand the importance of obtaining prompt care. However, it is essential to ensure that clinical documentation remains accurate and as complete as possible to facilitate the best patient care and safety.
