Bring ID, details of your medicines and any relevant medical records. For a child under 16, bring their passport or birth certificate. The accompanying adult must also bring ID. If they are not the child’s parent or legal guardian, they must bring a signed permission letter from the parent or guardian. - Is this an NHS walk-in centre?
